ExclusionWatch exists because of a pricing gap. Every organization that bills Medicare or Medicaid — down to a five-person dental office — is expected to screen its people against the OIG exclusion list every month. The vendors who automate that well charge thousands of dollars a year, priced for hospital systems. So small employers end up doing it by hand, doing it occasionally, or not doing it at all.
Who's behind it
ExclusionWatch is built and operated by Michael Koster through Kos Holdings LLC, an Oregon company based in Salem. It's a small, focused operation — which is exactly why the product can cost $49 a month instead of $4,900 a year. How our matching actually works
We publish our methodology because you're trusting it with a compliance obligation. Every month, right after OIG republishes the LEIE (about 80,000 active exclusion records), we screen every entry on your roster through tiered matching:
- NPI exact — an NPI on both sides that matches is definitive (100).
- Name exact — first and last name both match exactly (95); a matching date of birth strengthens it, a conflicting one effectively clears it.
- Name close — exact last name with a similar first name or initial (78).
- Name fuzzy — both names are close variants (68), so misspellings and transcription differences still surface.
- Business name — vendors and entities matched on reordered/fuzzy business names (85+).
Anything above our flag threshold goes to human review — yours. Per OIG guidance, a data-file match is only a potential match; the review workflow links you straight to OIG's official online search for SSN verification, and once you resolve a false positive we remember it so you never review the same pair twice.
What we don't do
We screen the federal OIG LEIE only. We don't yet cover state Medicaid exclusion lists or SAM.gov — if you need multi-list coverage today, our comparison page honestly lays out when an enterprise vendor is the better choice. We never collect Social Security numbers, we don't sell data, and we don't pretend a screening service is legal advice.
